Transitional Care is designed to assist older people transition from their recovery in hospital and address longer term care needs at home or in a Residential Aged Care Facility.
Our Transition Care program is a partnership between Australian Unity and Eastern Health. A range of services are provided including low intensity therapy such as occupational therapy, physiotherapy, nursing support, social work and/or personal care.
These services help improve independence, confidence and complete the recovery process to provide participants with optimal functional capacity.
Our Transitional Care Program operates from two facilities;
